Laser Root Canal Treatment

We all know that dental procedures are not painful and can give a comfort feeling to patients. But sometimes, these procedures are unpleasant and result in severe pain. Root canal is one of such painful procedure, which will be performed on the soft inner tissue of a tooth-known as pulp to remove the decayed or damaged part. The best way to relieve the pain and cause is through endodontics or root canal therapy.

Root canal procedure involves removing the diseased tissues from within a tooth and its roots, followed by disinfection. Dentists will insert filling material known as gutta percha, a natural occurring type of latex substance taken from tropical trees, into the canal to restore the natural tooth structure as long as possible. Although root canal procedures are successful, there are some cases in which the treatment does not yield expected outputs. In few cases, patient need to remove the tooth and replaced with an implant or bridge.
Laser technology changes the way you experience root canal therapy. Laser root canal therapy makes use a parallel beam of intense light to clean root canals. Laser therapy melts away debris comprising of bacteria and infection, known as smear layer of the root and cleans the root neatly when compared with traditional treatments. Laser dentistry can remove the infection within less time and pain.

Laser technology also minimizes possible risks from the root canal procedure. No need to worry about break off or getting stuck in the canal. The treatment can be carried out with minimal bleeding and can be used with minimal or no local anesthesia. Post-operative problems of conventional root canal treatment are practically eliminated-the laser reduces inflammation, swelling and discomfort in patient. In some cases, your dentist may combine both the laser and traditional hand pieces for optimal effects.

Re Root Canal Therapy

In few cases, root canal therapy fails to produce results as expected. After treatment, the patient might experience some problems or the treated tooth might not heal as expected. In such cases Re-root canal therapy is done.

Root canal retreatment removes the previous crown and packing material, cleanses root canals and the re-packs and re-crowns the tooth. Put simply, root canal re-treatment is similar to the actual procedure. The success rate of root canal retreatment is 75%.

Root canal treatments and retreatments is a good alternative to tooth extraction. If the tooth has a good bone support, a solid surface and healthy gums, then the tooth can be retreated instead of extracting. Root canal retreatment is less expensive when compared to other treatment procedures.

Why root canal retreatment?

The following are a few reasons why root canal therapy fails:

  • Cracked crown leaking filling material
  • Curved or narrow canals not treated properly
  • Restorative devices are not placed immediate after the treatment
  • New tooth decay
  • New fracture in the treated tooth
  • If saliva enters the restorative structure
  • Complex canal structures

Root canal retreatment procedure

Root canal retreatment procedure involves isolation of an affected tooth with a rubber dam. The dam protects the tooth during treatment from bacteria and saliva. The first step is to remove the placed crown and post. Followed by filling material and obstructions that block the root canals will be removed. All these will be removed using an ultrasonic handpiece. Tiny instruments are used to clean and reshape the root canals. X-rays will be taken to ensure the roots are properly cleaned.

If your dentist is confident that the root canals are cleaned completely, gutta-percha is inserted in that space. This rubbery material seals the canals to prevent accumulation of bacteria. At last, a temporary crown or filling is applied to tooth. Finally, a color matched permanent crown will be placed.

Apicoectomy

A root end surgery, known as apicoectomy, retrograde root canal treatment or root-end filling is an endodontic surgical procedure in which the root tip of tooth is removed and a root end cavity is prepared and filled with a biocompatible material. Usually, a microscope is used to carry out the procedure.
Apicoectomy is required when a root canal therapy is failed and a re-treatment is not successful or not advised. In simple terms, when an infection develops or consists after root canal treatment, or retreatment, Apicoectomy is performed. During root canal treatment, the canals are cleaned and any infected tissues are removed. But sometimes, even after root canal treatment, infected debris will remain the branches and cause re-infection. In Apicoectomy, the root tip or apex is removed along with the infected tissues. Later, a filling is placed to seal the end of the root.

Apicoectomy is not similar to a root resection, wherein entire root is removed rather than just the tip.

Initially, you will have a consultation with our dentists. Generally, endodontists will carry out this procedure. Dentists may take X-rays and will give an antimicrobial mouth rinse and anti-inflammatory medication prior to surgery. If you have high blood pressure or epinephrine in local anesthetics, let your dentist know at the time of consultation. The epinephrine will constrict your blood vessels to prevent bleeding near the area. Sometimes, you may feel your heart beat rate speed up after you receive the local anesthetic, but this will subside within few minutes.

Apicoectomy Procedure

The Endodontist will cut and lift the gum from the tooth so as to make the root easily accessible. The infected tissue will be removed with the few millimeters of the root tip. Dentist will use a dye that highlights cracks and fractures in the tooth. In case of tooth crack or fracture, it should be extracted and the apicoectomy cannot be performed.

In order to complete the apicoectomy, 3 to 4 millimeters of the tooth’s canal is cleaned and sealed. The cleaning is done with the help of microscope using ultrasonic instruments. Surgical microscope increases the chances of success because the light and magnification enables the endodontist to see the area better. Most procedures take between 30 to 90 minutes, based on the location of the tooth and complexity of the root structure.
